A day in the life of a Service Support Worker will involve:

  • Offering guidance and practical and emotional support (e.g., welfare inspections, shopping, and telephone calls) to service users
  • Providing reactive, focused, and flexible support to adults to facilitate an early discharge following a hospital stay.
  • Responding to referrals from community health professionals and physicians
  • Evaluate the requirements of service consumers, and develop and implement an individual support plan.
  • Assisting service users in the weeks following hospital discharge or to prevent readmission.
  • Accompanying an individual to a hospital appointment that they would not have been able to attend on their own, or assisting them in the process of obtaining prescriptions and purchasing.
